Practicum & Capstone Phase (6 Month Applied Phase)Â
After completing Tier 2, you’ll begin a 6-month, mentor-supervised practicum where learning becomes embodied through real-world supportive-care experiences.
You’ll meet with your mentor online throughout the practicum to ensure steady guidance, integration, and ethical support.
Includes:
• Minimum 200 hours of applied supportive-care exposure
• Flexible placements in community programs, hospice, seniors’ settings, or approved environments
• Regular online mentor meetings (weekly or bi-weekly depending on your placement and learning needs)
• Reflective integration using the APFCCM model
• Documentation practice appropriate for non-medical supportive roles
Capstone Project:
You’ll demonstrate your integration through one of the following:
• A reflective portfolio
• A case-based analysis
• A presentation
• A creative or narrative project
Certificates Earned:
• Tier 1 Certificate – Palliative Care Foundations
• Tier 2 Certificate – Advanced Supportive Palliative Care
• Comprehensive Palliative Care Practitioner Certificate (after the 6-month practicum & capstone)
